Bleaching of a patient was performed as part of clinical course in endodontics and aesthetic dentistry. A 23 year old man came to the clinic with yellowish teeth his teeth were cleaned and bleaching procedure was carried out.

Depigmentation procedure was performed at AHEAD Academy, New Delhi centre as a part of clinical course in periodontics. Such procedures are routinely carried out at our both centres, new Delhi and Mohali. Many extraction cases were performed last week at AHEAD Academy, New Delhi centre. These are few of the cases done by the students independently. An extraction case is one where a certain tooth was extracted to achieve fix orthodontic problems. Although rare, extraction is sometimes necessary, especially in cases of severe space deficiency.

In clinical class in oral surgery, principles of elevators were discussed. The students were elaborated the instruments used for tooth extraction. Elevator is an instrument used in minor oral surgical procedures. It is used to luxate the tooth from socket prior to the application of forceps and to expand bony socket there by aiding in tooth extraction, to reflect mucoperiosteum, luxate tooth , luxate and remove tooth when forcep engagement is not possible and to remove fractured or carious tooth. There are three mechanical principles which are applied to increase their mechanical advantage:
a) Lever Principle
b) Wedge Principle
c) Wheel and Axle Principle.

Orthodontics extractions were performed by a student of present batch, Dr. Arpit. He is now proficient in doing RCTs and extractions. Extraction is necessary part of the treatment in some cases, but in others it is contraindicated. Before extractions are considered, an exhaustive study of the case must be made by a clinical examination of the patient, paying particular attention to caries or restored or traumatized teeth which might subsequently give rise to symptoms. Radiographs should be scrutinize to ensure the presence of all teeth. The degree of dental discrepancy (crowding) and patient age play important roles in the decision to treat a malocclusion (poor bite) with or without the extraction of permanent teeth. A patient who presents in the permanent dentition with a severe enough discrepancy in arch length (the size of the teeth versus the size of the jaw) might very well require extraction treatment. There are also situations in which the upper front teeth need to be retracted in order to reduce the horizontal space (overjet) between the upper and lower front teeth.

Classes at AHEAD Academy

Online live classes are regularly conducted at AHEAD Academy, New Delhi and Mohali. These classes are attended by students preparing for MDS and foreign exams. Today online live lecture was held at AHEAD Academy, New Delhi. The topic of discussion was “fungal infections and skin pathology”. Fungal infections are becoming more frequent because of expansion of at-risk populations and use of treatment modalities that permit longer survival of these patients. Some of the changes in endemic fungal infections can be attributed to climate changes, extension of human habitats, ease of travel, and shifting populations. Today, fungi previously considered nonpathogenic, including mucoraceous genera (formerly called zygomycetes) and a variety of both hyaline and dematiaceous molds, are commonly seen in immunocompromised patients. The class was attended by many students online. Two students who are preparing for DCM (Dental Council of Mauritius) exam also attended the class.

                                     
In today’s clinical class in prosthodontics at AHEAD Academy, New Delhi, taken by senior faculty many topics were discussed. The importance of occlusal records was emphasized. The students were also shown different prosthodontic materials. Interocclusal records are the means whereby the inter-arch relationships are transferred from the mouth to an articulator. Interocclusal records are the most important maxillomandibular records used to transfer interarch relationships from the mouth to an articulator. Accurate interocclusal records minimize the need for intraoral adjustments during prosthesis delivery and are essential in providing high-quality fixed restorations and reducing overall treatment time and cost.

Ideal requirements of bite registration material are:
• The materials should offer limited resistance before setting to avoid displacing the teeth or mandible during closure, whereas after setting, it should be rigid or resilient, with minimal dimensional change.
• It should be easy to manipulate with no adverse effects on the tissues involved in the recording procedures.
 • It should accurately record the incisal and occlusal surfaces of teeth.
• A very important feature - it should be verifiable.

A case of Root Canal Treatment (RCT) was started by a student of our new batch, Dr. Tanu. The pulp was removed by the student. A root canal is a treatment to repair and save a badly damaged or infected tooth. The procedure involves removing the damaged area of the tooth (the pulp), cleaning and disinfecting it and then filling and sealing it. The common causes affecting the pulp are a cracked tooth, a deep cavity, repeated dental treatment to the tooth or trauma. The term "root canal" comes from cleaning of the canals inside the tooth’s root

Patients frequently develop problems with impacted third molar (wisdom) teeth. Impacted wisdom teeth are fairly common. Although some people never have any trouble with them, impacted wisdom teeth can cause problems. An impacted wisdom tooth can sometimes trap food, plaque, and other debris in the soft tissue around it, leading to swelling, gum tenderness, cavities, and bad breath. There may be no symptoms of a fully impacted tooth. Symptoms of a partially impacted tooth may include: Bad breath Difficulty opening the mouth (occasionally) Pain or tenderness of the gums or jaw bone Prolonged headache or jaw ache Redness and swelling of the gums around the impacted tooth Swollen lymph nodes of the neck (occasionally) Unpleasant taste when biting down on or near the area Visible gap where a tooth did not emerge.
There are two main types of tooth extraction, “simple,” and surgical. With a simple extraction, the tooth is removed using the same basic tools as with a surgical extraction, but without the need for any type of incision to the gingiva, nor segmenting of the tooth to aid in its removal. In contrast, a surgical extraction would require at least one of these two methods of enhanced extraction. This dental procedure code covers surgical extraction, and can also be referred to as an “open” extraction.

In Endodontics Class at AHEAD Academy

In endodontics class at AHEAD Academy, New Delhi, midline diastema closure was discussed. The steps were explained in detail to the students. They were motivated to practice on more number of patients. The midline diastema is a space (or gap) between the maxillary central incisors. The space can be a normal growth characteristic during the primary and mixed dentition and generally is closed by the time the maxillary canines erupt. A midline diastema usually is part of normal dental development during the mixed dentition. However, several factors can cause a diastema that may require intervention. An enlarged labial frenum has been blamed for most persistent diastemas, but its etiologic role now is understood to represent only a small proportion of cases. Other etiologies associated with diastemas include oral habits, muscular imbalances, Physical impediments, abnormal maxillary arch structure, and various dental anomalies. Effective diastema treatment requires correct diagnosis of its etiology and intervention relevant to the specific etiology. Correct diagnosis includes medical and dental histories, radiographic and clinical examinations, and possibly tooth-size evaluations. Appropriate treatment modalities have been described. Timing often is important to achieve satisfactory results. Removal of the etiologic agent usually can be initiated upon diagnosis and after sufficient development of the central incisors. Tooth movement usually is deferred until eruption of the permanent canines, but can begin early in certain cases with very large diastema.

Clinical class in endodontics as part of clinical certification course at AHEAD Academy

Clinical class in endodontics was conducted at AHEAD Academy, New Delhi centre. The topic of discussion was canal negotiation and working length determination. The students were explained the technique of access opening in detail. They were shown the root canal anatomy of all the teeth. The shape of all canals was demonstrated in detail. They were shown the procedure step-by-step on extracted and typodont teeth.

Clinical Certificate course in prosthodontics

Clinical class in prosthodontics was held at AHEAD Academy, New Delhi centre. Veneers were discussed in detail. Veneers are thin pieces of porcelain used to recreate the natural look of teeth. They also provide strength to the tooth. It is used in teeth with poor colour, shape and contours. Veneers are a good choice for fractured teeth, gaps between teeth and bite related problems smile designing was elaborated. A smile makeover is the process of improving the appearance of the smile through one or more cosmetic dentistry procedures like veneers, composite bonding, implants and whitening. Zirconia crowns and their properties were taught to the students. The trade names and market price of crowns and resin cements were told to the students. The technique was then demonstrated to the students in detail.





Clinical course in prosthodontics is going on at New Delhi centre. New students were given a demonstration of crown preparation. Before giving a crown, a tooth’s upper part, crown has to be reduced to make space for crown. The students were shown the step-by-step procedure for crown preparation. Clinical Course in General Dentistry

At Ahead Academy, we treat patients from foreign countries also. Last week, a 56 years old man visited the clinic from Afghanistan. The patient had pain in his left lower molar. His tooth was extracted and he was relieved of pain. Many such patients visit AHEAD Academy, New Delhi and Mohali.






  
 A new student of clinical course in endodontics completed obturation of lower molar with sealer puffs. When using a warm gutta percha hydraulic obturation technique it is routine to observe a sealer puff associated with a portal of exit. Surplus sealer after filling is well-tolerated and research supports its biocompatibility. Advantageously, zinc oxide and eugenol form zinc eugenates which block prostogladins and pain. Patients are more comfortable post-operative when a sealer sets rapidly and completely in the periradicular tissues. Classic sealers take 24-48 hours to set, a more recently marketed sealer, AH Plus, sets in approximately 6 hours, and Kerr Pulp Canal Sealer sets in less than 2 hours.




  
 A case of splinting was performed at AHEAD ACADEMY, New delhi. The patient came with loose teeth in front region. He was happy with his appearance and esthetics. Periodontal stabilization splints are used to stabilize teeth which have become loose as a result of losing the supporting bone around them to periodontal disease, a condition known as secondary occlusal trauma. Frequently the problem is complicated by heavy bite stress. If the teeth have lost more than 40% of the supporting alveolar bone around them, a diagnosis of “severe” periodontal disease is made. Most often, teeth that are loose (mobile) enough to require splinting have that diagnosis. However, if the tooth roots are abnormally short or thin, or if the bone around them is not particularly dense, the teeth may be loose when less than 40% of the volume has been lost.

A 34 year old patient came with pain in lower front teeth and fractured upper front teeth. Root canal treatment (RCT) was planned was lower front teeth, later on followed by crowns. The upper front teeth were restored with composite fillings that are tooth coloured. The patient is extremely glad with his appearance and smile. Many such patients visit AHEAD Academy with their relatives and benefit from the facilities. For many such patients, their appearance has been improved drastically. Most commonly used filling material is the composite filling material as it is tooth coloured associated with and adhere well to the tooth. In this minimum tooth material is lost as the filling has mechanical bonding with the tooth. It is an esthetic filling and has good strength. They come in different shades so the shade matching can be done easily.

Treatment of patients at a reasonable cost at Karol Bagh

Clinical class in endodontics was held at AHEAD Academy, New Delhi centre. The topic of discussion was post and core. Students were explained the procedure in detail and then demonstrated on typhodont teeth. The step by step procedure was then shown to the students. Posts (or dowels) can generally be divided into two main subgroups, depending on how retention is achieved. Active posts derive their primary retention directly from the root dentine by the use of threads. Passive posts on the other hand gain retention as their name suggests by passively seating in close proximity to the post hole walls, and rely primarily on the luting cement for their retention.1 Each post type can further be subdivided according to its general shape, that is whether it is tapered or parallel sided. In general, active posts are more retentive than passive posts of a similar configuration, and parallel-sided posts are more retentive than tapered posts. Post choice should therefore be dictated by each clinical situation. Regular classes are held at both the centres, New Delhi and Mohali. All students are welcome to attend these classes.





Depigmentation procedure is routinely performed at both the centers of AHEAD Academy. A 23 years old male came to the clinic with blackish gums. His gums were treated and changed to pinkish colour. Various methods such as gingivectomy with free gingival autografts and acellular dermal matrix allografts, electrosurgery, cryosurgery, abrasion with diamond bur, and laser therapy have been used in the treatment of gingival melanin depigmentation with varied degrees of success. Healthy gingival and its appearance are essential factors for an attractive smile and removal of hyperpigmented gingiva is the necessity for a better and beautiful smile. The pigmentation becomes an esthetic concern in patients with a high smile line.

Case of fused molars

A patient came to the clinic with pain in his upper right region of jaw. His molars were fused together. The extra tooth was removed and the patient was relieved of pain.Fused/geminated teeth may cause esthetic, spacing, periodontal, eruption and caries problems.Fusion is an abnormality of tooth development, defined as the union at the enamel, dentine, cementum or pulp level during different stages of odontogenesis of two or more separately developing contiguous tooth germs. When fusion takes place, the pulp chambers and root canals may either be joined or separated, depending on the stage of tooth development at the time of union. The process of tooth fusion involves epithelial and mesenchymal germ layers, usually resulting in irregular tooth morphology.



Many flap surgery cases were operated at AHEAD Academy, New Delhi. These cases were performed by students of clinical course in periodontics.Flap surgery is a technique in plastic and reconstructive surgery where any type of tissue is lifted from a donor site and moved to a recipient site with an intact blood supply. This is similar to, but different from, a graft, which relies on growth of new blood vessels.The purpose of periodontal surgery is to treat deformities and tissue loss created by the disease process, eliminating “pockets” of diseased tissue in order to create and maintain periodontal health. Techniques have been developed to deal with the gingival (gum) tissues and underlying bone. The objectives of periodontal flap surgery are to:
  1. Eliminate or reduce pockets.
  2.  Regenerate periodontal tissues and their (re)-attachment to the teeth.
  3. Create more normal periodontal form, function, and aesthetics;
  4. Promote an environment more conducive to good oral hygiene practices and professional maintenance care.

Class of suturing as part of clinical course

Suturing techniques were demonstrated to the new batch of students. Different techniques were practiced by the students after the demonstration.There is a need for students to be able to make a logical decision regarding which suture materials to use in a given clinical situation because the choice of wound-closure materials may make a difference in wound healing and the ultimate functional and cosmetic result obtained.Basic suturing techniques were also reviewed. Knowledge of multiple suturing techniques provides versatility, individuality, and optimal closure of surgical wounds.Techniques that must be mastered include good eversion of skin edges, avoiding suture marks, maintaining uniform tensile strength along the skin edges, and precise approximation along skin edges.

Clinical class in endodontics was conducted at AHEAD Academy, New Delhi. The topic of discussion was veneers and laminates. The types of veneering were explained to the students..The step-by-step procedure of veneering was elaborated by the staff. The new students were explained the basic chair positions while operating a patient on dental chair.Porcelain veneers (also referred to as dental laminates), are wafer-thin shells made out of dental ceramic that are bonded onto the front side of teeth. Laminate veneers are restorations which are envisioned to correct existing abnormalities, esthetic deficiencies and discolorations.Laminate veneer restorations have two different types: direct and indirect laminate veneers. Direct laminates are applied on prepared tooth surfaces with a composite resin material directly in the dental clinic. Absence of necessity for tooth preparation, low cost for patients compared with indirect techniques and other prosthetic approaches, reversibility of treatment and no need for an additional adhesive cementing system are some advantages of this technique. Intraoral polishing of direct laminate veneers is easy and any cracks or fractures on the restoration may be repaired intraorally and marginal adaptation is better than that of indirect laminate veneer restorations.

Preparation for AIIMS MDS Entrance Examination at AHEAD Academy, New Delhi and Mohali

The examination calendar of AIIMS PG 2017 for July session has been released by the exam conducting authority, AIIMS, New Delhi. Candidates are advised to check below for the important dates and mark their calendars accordingly:

The eligibility criteria of AIIMS PG 2017 for July session is given below. It is advised to read the eligibility criteria thoroughly before filling up the application forms. Also, candidates who fail to meet the requirement will not be given admission:
  • Candidates must have an MBBS degree for MD/MS and BDS degree for MDS from a recognized institution recognized by Medical Council of India/Dental Council of India
  • Candidates must have completed the 12 months of rotating internship on or before April 30, 2017
  • Candidate belonging to general category must have secured a minimum of 55% aggregate in MBBS, while it is 50% for candidates belonging to SC/ST categories
The online registration for AIIMS PG 2017 July session will begin from March 8, 2017. The application forms will be available in online mode only and no other mode was accepted. The last date to fill the online application form is March 29, 2017.
The application fee amount of AIIMS PG 2017 was Rs.1000 for General and OBC candidates and Rs.800 for SC/ST candidates. Along with the application fee amount candidates have to pay the transaction charges. The fee have to be remitted through net banking, debit card and credit card 
AIIMS PG 2017 is a Computer Based Test (CBT). The question paper will be based on the syllabus prescribed by the exam conducting authority, AIIMS, New Delhi. The online question booklet will have questions Pre-Clinical, Clinical and Para-Clinical subjects. Candidates can check below for the highlights of AIIMS PG 2017 exam pattern: 
Exam pattern for MDS
  • Duration: 1 hours 30 minutes (90 minutes)
  • No. of questions: 90 Objective based multiple choice questions
  • Marking Scheme: 1 marks for each correct response
  • Negative Marking: 1/3rd of the marks will be deducted for each incorrect response
  • Syllabus: Exam will be based on the syllabus taught during BDS
The admit card of AIIMS PG 2017 will be available for those candidates only whose application forms will be accepted by the exam conducting authority, AIIMS, New Delhi. The admit card will be available in the month of April, 2017, under candidate’s login. AIIMS PG 2017 July session admit cards consist of candidate’s name, examination details, examination centre address etc.
The result cum merit list of AIIMS PG 2017 will be declared in online mode on May 13, 2017. The result will be declared for those candidates only who have secured a minimum of 50 percentile in the entrance exam irrespective of their category.
The cut off is the minimum marks required to obtain in AIIMS PG 2017 in order to qualify the entrance exam.  The cutoff marks have been prescribed by the examination authority, AIIMS, New Delhi. According to the official brochure, candidates have to secure a minimum of 50 percentile in AIIMS PG 2017 irrespective of their category. Based on the cut off marks, the result cum merit list will be prepared and candidates will be called for AIIMS PG 2017 counseling.

AIIMS PG 2017 July session is conducted for providing admissions to as many as 183 MD/MS and 7 MDS seats in AIIMS New Delhi, AIIMS Bhopal, AIIMS Bhubaneswar, AIIMS Jodhpur and AIIMS Rishikesh. The number and distribution of seat may vary depending upon the directions from Competent Authorities.

The counseling of AIIMS PG 2017 will be conducted in three rounds including the open selection. The first counseling will be conducted on June 5, 2017, followed by second and open round on June 13 and August 24, 2017, respectively. For each category, the number of candidates called for counseling will be eight times the number of seats. The counseling will be done purely on the basis of AIIMS PG 2017 merit list. The candidate in order of merit will exercise their choice of subject in accordance to the availability of seats.
